All athletes must have an up-to-date or alternate year physical form and an emergency card turned into the office before they are eligible to practice with the team. Please click on the link below, print out the forms, and turn them into your school office. Every AW-K swimmer and diver needs to pay a $50 fee in order to participate on the team. Please make checks out to Appleton West Boys Swimming & Diving and bring or mail them to the business office at Appleton West.
